Transcript Request Procedures

A student may obtain a copy of his/her own academic record in person by completing a transcript request form or by writing the Office of the Registrar at any campus of Indiana University. Requests must either be signed by the student of former student or accompanied by a release signed by the individual whose transcript is being requested.
